Just making it official here, though you all know I've been consistently lax in this.
Some Immediate Interrupts that prevent attacks have a specific triggers, with various examples as follows:
eg. Ranger's Disruptive Strike has a trigger "You or an ally is attacked by a creature"
eg. Wizard's Shield has trigger "You are hit by an attack"
eg. Paladin's Price of Cowardice "An enemy marked by you makes an attack that does not include you as a target"
TECHNICALLY speaking by 4E Rules, you would have to decide whether to use the Interrupt
a) If the trigger is "Attack" and not "Hits", BEFORE the DM rolls the d20 Dice to Hit / Miss.
a) Otherwise, generally you would have to interrupt on a "Hit" BEFORE Damage is rolled
b) BEFORE the Effect of the Power hitting you / your Ally is made known
But for All My Games, just to make things clear :
You can apply these things AFTER the DM rolls d20 To Hit, AFTER the DM rolls damage, AFTER the DM states the effect, AFTER the Player says something like "Oh I'm down / dead"
( you don't have to wait that long though, if you already know you want to interrupt, or you already know what nasty effect the power has )

Note that if asked, I will tell Players what the d20 roll is. However, if I already forgot what was the Roll (particularly for AoEs), I will just treat it as if it were a 13 or high enough to hit, whichever is higher.
