I found the following Entries in Glossary, looks like our "House Rule" of Resistance first followed by Insubstantial looks correct.
Half Damage
"Some powers deal half damage when they miss, and some effects, such as the weakened condition, cause damage to be halved. When a power or other effect deals half damage, first apply all modifiers to the damage, including resistances and vulnerabilities, and then divide the damage in half (rounded down).
Example: Albanon the wizard is weakened and uses ice storm, which deals 16 cold damage to a fire giant that has vulnerable 5 cold. First add 5 to the 16 cold damage for the vulnerability, giving a result of 21. Then divide that result in half for the weakened condition. The giant ends up taking 10 cold damage.
Insubstantial
Some creatures, such as wraiths, are naturally insubstantial, and some powers can make a creature insubstantial. While a creature is insubstantial, it takes half damage from any attack that deals damage to it. Ongoing damage is also halved."
Note the Glossary entry for Insubstantial is
Insubstantial
"When a creature is insubstantial, it takes half damage from any damage source, including ongoing damage. See also half damage."
This implies that Insubstantial results in half damage, and is not separate from Insubstantial.
Hence we have
a) Weakened Enemy results in half damage.
b) Some Enemy Powers that miss results in half damage.
c) Certain Player powers and/or feats allow Insubstantial which result in half damage.
d) Certain Player items, under certain conditions, result in half damage.
Theoretically, none of these stack.
However, I think under some condition it might stack, roughly separated into Player and Enemy reasons.
So as a House Rule, a) and b) don't stack, c) and d) don't stack, but the two categories stack with each other.

Meantime, as a separate House-Rule (pre-informed Nerf)
Player-related Half-Damage (ie c & d) no longer stacks with Resistance
This also means Insubstantial (under c) does not stack with Resistance.
You can choose to apply either, but you cannot apply both.
