( All magic items can be upgraded per 5 levels, even if the numerical benefit does not change )
When you have a permanent magic item that is more than 10 levels below your level (edit, during update to change to 10 levels or more below), when you first become bloodied during the encounter, the magic item breaks. If you took damage during the encounter but did not get bloodied, make an unmodified saving throw after the encounter, and if you fail, the magic item breaks. When you have more than 1 unbroken item (that can become broken), randomly choose 1 such item to become broken.
This applies as long as you are benefitting from the item, even if it is not visibly equipped on you. It also applies if you are holding, have it "ready to equip/put on" or other such contrivances that would expose it in contact with your enemies. However, if it is stashed in your backpack and did not benefit from it at any time during the encounter, it does not become broken.
A broken magic item confers no benefits and cannot be sold for gold.
It can be repaired via an Enchant Magic Item ritual or at a smith and by consuming/paying 50% of its original value.
A broken magic item can also be upgraded by 5 levels (by consuming/paying the difference in item value, typically 4 times its original value), and doing so repairs the damage with no extra cost. It also helps ensure the item can withstand the damage from the more powerful threats that are faced.
Specialised Training never becomes broken.

It was pointed out that Adept Conserver and Reinforced Gear both exist. For now, treat Adept Conserver as deleted, and Reinforced Gear to read as follows.
Reinforced Gear Choose a specific magic item that you have equipped in either your Belt or Boots slot. For purposes of being broken, it is always treated as if it is less than 10 levels below you
